Remote Access (VPN) Request

Remote Access VPN Requests must be submitted by the a person's manager, HR or responsible party. 

For a new staff, staff augmentation or vendor, please submit a New User Setup Request first.

For new VPN access, please fill out and attach one of the following forms of the following forms is required:

• For Staff, please fill out and attach a Employee VPN Request form.
• For Staff Augmentation or Vendor Access, please fill out and attach a Vendor Access Request (No VPN) or a Vendor VPN Request form.

NOTE: If a form has previously been submitted and you only require an extension of access, you do not need to submit a new form, just indicate the new end date when submitting this request.
